Insurance Requirements

Workers’ Compensation Insurance – coverage for the Worker’s Compensation statute or jurisdiction where work is preformed. Employers Liability limits of $1,000,000 each accident, $1,000,000 each disease-each employee and $1,000,000 disease policy limit.
Commercial General Liability insurance including bodily injury, property damage, personal and advertising injury with minimum limits of $1,000,000 combined single limit each occurrence and $2,000,000 aggregate.
General Liability insurance will contain the following items:
- No residential or subcontractor limitation(s) added to the policy.
- Coverage must be provided on an occurrence type of policy.
- Deductibles not to exceed $10,000
Automobile Liability with a minimum $1,000,000 combined single limit each accident.
Umbrella Liability covering General Liability, Automobile Liability and Employers Liability for not less than $1,000,000 per occurrence and aggregate.
Pollution Liability Insurance:
- Limits: $1,000,000 per occurrence and aggregate
- Coverage: pollution including mold
- Deductible: not to exceed $10,000
Subcontractors property including equipment (including rented equipment), supplies, and material. Subcontractor will be responsible for these items and provide insurance for these items.
All insurance limits set herein are a minimum requirement. Limits are subject to change depending on requirements set forth by each Purchase Order as it pertains to each job location.
The Subcontractors Liability, General Liability and Auto Liability policies above shall name Owner (and, if applicable Owners Manager and/or Architect) and MAM LLC as an additional insured on a primary and non-contributing basis regarding Subcontractors operations. Subcontractor will provide a waiver of subrogation applicable to all coverage evidenced by the Certificate of Insurance in favor of Owner and Mid America Metals, unless prohibited by statue or regulations. Subcontractor’s failure to provide evidence of insurance as required herein shall be deemed a material default under the terms of this Agreement
Subcontractor shall provide Mid America Metals with a certificate of insurance within three (3) days of contract being awarded. Mid America Metals shall have the right to terminate this Agreement if any of the insurance policies or coverage’s required hereunder are cancelled or reduced as set forth herein.
Subcontractor’s insurance carrier must be permitted to do business in the state in which the work is to be performed and holding an A.M. Best’s rating of A-VIII or better.
